Form 5106- Importer ID Input Record

Since your order is over $2500, US customs requires you to complete Form 5106 (Importer ID Input Record) to process it.

This form is needed to identify importers (in this case, you—because the package is coming to the US from outside the US and is for you) and ensure your shipment clears customs smoothly.

Here’s what you need to do:

Once completed, you can do one of the following:

• Option 1: Send the completed form to us, and we will forward it to FedEx for you.
• Option 2: Contact your local FedEx branch, provide them with your tracking number, and ask where to send the form.

After this step, FedEx will handle the delivery of your shipment.
